Outdoor Usage Permit Request Form

Please Note:
Approval of this outdoor use permit is required prior to usage and must be presented to Town officials if requested.

This permit allows for outdoor usage in the approved area; in a manner that complies with all federal, state, and local laws, rules, and regulations.
This permit is for outdoor usage in an area that includes no restroom facilities, trash receptacles, or furnishings.
The Contracting Party must maintain its own general liability insurance policy. Usage is at your own risk. Acceptance of this permit and beginning and ending on the date and time of the approved permit, the Contracting Party shall fully indemnify and hold the Town harmless from any and all claims, actions, causes of action, damages, compensatory damages, punitive damages, injuries to persons or property, costs, expenses (including but not limited to court costs and attorneys’ fees, interest and penalties), and from any and all costs or damages to the Town, either directly or through third-party claims, arising out of or claimed to arise out of any or all acts or omissions of the Contracting party arising out of or related to this Agreement and matters and things referenced herein.
